The ‘Issuance of a certificate of the testator's last place of residence and his/her family composition on the date of death‘ administrative procedure is performed pursuant to subparagraph 1.3.5 paragraph 1.3 of the List of Administrative Procedures by Government Agencies and Other Organizations at the Request of Citizens approved by Decree of the President of the Republic of Belarus No. 200 dated April 26, 2010.

Authorized authority for the administrative procedure — an organization that carries out the recording, calculation and accrual of payment for housing and communal services and payment for the use of residential premises; an organization that operates the housing stock and (or) provides housing and communal services; another organization that carries out the accrual of payment for housing and communal services and payment for the use of residential premises (in relation to residential premises for which the recording, calculation and accrual of payment for housing and communal services and payment for the use of residential premises are carried out by this organization); an organization that provided the residential premises, the Council of Deputies (an executive committee) of the village, settlement, city (towns under district jurisdiction), district.

The term of validity of the administrative decision taken as a result of the administrative procedure is unlimited

To apply for the administrative procedure, the interested person should log in to the Unified Portal of E-Services (https://e-pasluga.by) with strict authentication through the Unified System of Identification of Individuals and Legal Entities (ES IFYUL) (using a valid personal key, the public key certificate of which was issued by the Republican Certification Centre of the State Public Key Management System, biometric identity documents, as well as other means of strict authentication available in the ES IFYUL) or a unique identifier, select administrative procedure 200.1.3.5 from the list of available administrative procedures and click on the "Order" button, select the authorized authority, fill in the information required for the administrative procedure and send the documents to the authorised authority for consideration and administrative decision.

 

The result of the administrative procedure will be available to the interested party in his/her personal electronic cabinet on the Unified Portal of E-Services in the ‘My Services’ section when viewing the previously submitted application

— on refusal to accept an application from the interested party;

— on the implementation of the administrative procedure;
