The administrative procedure “Amendment to the licence to perform forensic examinations” is carried out on the basis of Resolution of the Council of Ministers of the Republic of Belarus No. 548 dated September 24, 2021 “On Administrative Procedures Carried out in Relation to Business Entities”.
List of required documents:
1. An application for the licence amendment according to the Annex 2 to the Regulations on the procedure for submitting and lists of documents and (or) information necessary for making decisions on licensing issues, requirements for submitted documents and (or) information, approved by Resolution of the Council of Ministers of the Republic of Belarus No. 154 dated February 27, 2023 (hereinafter referred to as the Regulations), indicating the information specified in sub-clause 35.1 of clause 35 of the Regulations;
2. A copy of the deed of transfer, separation balance sheet, constituent documents in the case of a merger or acquisition, or any other document or its copy, which clearly indicates the reorganization of the licencee - legal entity and the transfer of the licence to the legal entity - applicant (when applying for the licence amendment due to the reorganization of the licencee - legal entity);
3. Copies of the constituent or other organizational and administrative documents of the licencee - legal entity (the legal entity to which the licence has been transferred) defining the status of the separate subdivision of this legal entity in which the licencee (the legal entity to which the licence has been transferred) intends to carry out the licenced activity (in the case of a licence the due to a change in the list of separate divisions, including their name and/or location, or the reorganization of the licencee -legal entity).
4. Copies of documents confirming ownership or other legal basis for the material and technical resources and conditions necessary to conduct forensic examinations of the declared types (subtypes) of forensic examinations (in the event of a change in the information provided for in Article 134 of the Law of the Republic of Belarus “On Licensing”, as well as in the case of the licence amendment due to the reorganization of the licencee –legal entity): must comply with the requirements provided for in the second paragraph of sub-clause 35.2 of clause 35 and the third paragraph of clause 36 of the Regulations;
5. Information about the employees of the licencee - legal entity - who will perform (is performing) the licenced activity (in the event of a change in the licence due to a change in the information specified in the third paragraph of Article 134 of the Law of the Republic of Belarus “On Licensing”, as well as in connection with the reorganization of the licencee - legal entity), in relation to each of the declared types (subtypes) of forensic examinations;
6. Information about the location(s) where the licenced activity is carried out, the types (subtypes) of forensic examinations for which the licenced activity is carried out (in the event of a change in the licence due to the reorganization of the licencee - legal entity);
7. A copy of the diploma (or duplicate) of higher education of the head of the legal entity (in the event of a change in the licence due to the reorganization of the licencee - legal entity);
8. An extract (copy) from the employment record book of the head of the legal entity for the last three years prior to the date of application for a licence change (in the event of a licence change due to the reorganization of the licencee - legal entity);
9. A copy of the document establishing the tariffs for the types (subtypes) of forensic examinations performed (in the event of a licence change due to the reorganization of the licencee - legal entity).
The interested party must log in to the Unified Portal of Electronic Services (https://e-pasluga.by/) with strict authentication through the Unified System for Identification of Individuals and Legal Entities (using a valid private key, the public key certificate of which is issued by the Republican Certification Centre of the State System for Management of Public Keys for Electronic Digital Signatures Verification of the Republic of Belarus, biometric identity documents, as well as other methods of strict authentication available in the Unified System for Identification of Individuals and Legal Entities).
Select administrative procedure 548.12.3.2 from the list of available administrative procedures. Read the description of the administrative procedure, fill in the electronic application form with the necessary documents attached, and send the application to the authorized authority for consideration and administrative decision-making.
The result of the administrative procedure in electronic form will be available to the interested party in their personal electronic account on the unified portal of electronic services in the “My Services” section when viewing the previously submitted application

The result of the consideration of the application for the administrative procedure is an adopted administrative decision:
- on refusal to accept the application;
- on the implementation of the administrative procedure;
- on refusal to implement the administrative procedure
