Registration status (applicant, unemployed)

Address of registration at a place of residence

Identification (personal) number

Number of identity document

Date of birth

Period of registration as an unemployed person or as an applicant (Date from... — Date to...)

Reason for removal from the register

Number of weeks during which a citizen was entitled to benefits

Number of weeks and days during which a citizen received benefits (line)
